• 3-month minimum commitment required.

• Membership automatically renews monthly until canceled.

• Cancellation requires 7 days' written notice before the next billing date.

• Cancellation before completing 3 months is subject to a fee equal to one monthly VIP Bank payment ($150 Revive / $250 Elevate / $420 Elite).

• Unused VIP Bank credits roll over.

• Credits, discounts, and benefits are member-only and non-transferable and cannot be redeemed for cash.

• Discounts apply to eligible, full-priced services and cannot be combined with promotions, packages, or other discounts.

• VIP benefits apply to in-office services only; mobile IV, travel, and after-hours fees are excluded.

• After cancellation, membership discounts and perks will remain active through the end of the member’s current paid billing period. After the billing period ends, all membership discounts and perks will terminate. Any remaining eligible VIP Bank balance may still be used toward services at regular à la carte pricing.

• If a canceled member has a remaining VIP Bank balance and wishes to reactivate their membership to receive VIP discounts on that balance, a $50 reactivation fee and a new 3-month minimum commitment will apply. If there is no remaining VIP Bank balance, the member may re-enroll without a reactivation fee, subject to a new 3-month minimum commitment.

• Membership does not guarantee eligibility for treatment. All medical and aesthetic services are subject to provider assessment and approval.

• Revived Body's standard appointment cancellation and no-show policies still apply.

Revived Body reserves the right to modify VIP Bank benefits, exclusions, and program terms with appropriate notice.
