A minimum commitment of 3 months is required

If canceling before completing 3 months, a one-time cancellation fee equal to one full month’s membership price will apply. (e.g., $150 for Revive, $230 for Elevate, $410 for Elite)

Membership auto-renews monthly unless canceled in writing with at least 7 days’ notice before the next billing cycle.

Unused monthly services may roll over and must be used within 3 months.

Monthly complimentary services (“free gifts”) are for members only and cannot be shared, transferred, exchanged, or substituted for cash or other services.

Members are welcome to bring guests; however, membership discounts and complimentary services do not apply to guests.

Discounts apply only to full-priced, à la carte services and cannot be combined with other promotions or packages.

Membership benefits apply to in-store services only and do not apply to mobile IV appointments.
